Tibial Internal Rotation: The Missing Piece in Knee Pain, Pronation and Ankle Dorsiflexion

ankle dorsiflexion flat feet foot mechanics knee pain medial hamstrings pronation tibial internal rotation Oct 17, 2026

Tibial internal rotation is an important component of knee flexion, ankle dorsiflexion and pronation.

Two common patterns

You can have flat feet that turn out with knees that face forward. Or you can have feet that face forward (usually with more rigid, high arches) and knees that face in.

Where the medial hamstrings come in

The medial hamstrings can actively help with the proximal internal rotation. I usually do this in conjunction with:

  • drills to improve pronation and supination of the foot
  • exercises that help with glute control
  • exercises for medial quad strength
